Activities Forms
Once all necessary forms have been submitted to the Activities Office, located in the main school office, the Activities Office will notify your coach that you are cleared to begin practice. Student athletes with incomplete paperwork will not be allowed to begin practice until it has been submitted in entirety.

Fall Sports Letter

Sport Registration Form/Athletic Eligibility Brochure (pdf)
Must be submitted once each year by each student athlete

Physical Exam (pdf)
A physical that has been completed within the last three years must be on file with the Activities Office. If your physical will expire during the sports season for which you are registering, you must provide a current physical BEFORE you can begin your next sports season within the school year.

Concussion Management Addendum Brochure (pdf)
The State of Minnesota has passed a new law surrounding education of concussion and athletes. The Concussion Management Addendum Brochure is a new form that parents and athletes are to read and sign. This is in addition to the eligibility statement and physical forms.

The MSHSL Sports Medicine Advisory Committee and Holy Family highly recommends that every student-athlete and parent successfully complete the Head Up Concussion in High School Sports course. This course can be accessed at www.cdc.gov/concussion.
